What do I need to bring for my appointment?
Please always bring your picture identification and your most current insurance card to any appointment, not just your first visit. We also ask that you print out the needed paperwork from our website, fill it out, then bring it to your appointment. If you are transferring care from another practice, we ask that you transfer your records from that practice to us prior to your appointment.

How early do I need to arrive for my appointment?
If you are a new patient, and you haven’t completed the new-patient paperwork, please arrive 30 minutes early to complete it. If you have printed out your new-patient paperwork on our website and have it completed, arrive about 15 minutes prior to your appointment. This also applies to established patients.
What insurances do you take?
A list of accepted insurance carriers can be found on our Insurance page. However, insurance plans can change, so please call us at (972) 542-8884 to verify that your plan is accepted.
When can I expect my results?
The normal turnaround for results is two weeks. But, we encourage you to sign up for a Patient Online Access account, the fastest way to receive your results — usually in less than the normal two weeks. You receive an email notification when your results are ready, and then you’ll be able to log in and view the results right away. Your Patient Online Access account is also the easiest way to communicate with your provider. You can send questions through your POA account directly to your physician. Without a POA account, normal results are mailed to you, and the expected turnaround will be the full two weeks.
It’s been two weeks, and I haven’t received my results. What do I do next?
Check your email and your spam/junk folder. You may have received an email notification that was blocked by your spam filter. Lack of results may also mean that we haven’t yet received them. If it has been two weeks, please call our office at (972) 542-8884 for the status.
I requested a refill on my medication; when can I expect it to be ready?
Typically, medication refills take just two business days. If it has been longer, there may be a problem, like a visit is needed first, or the request was not appropriate. Please call us at (972) 542-8884.
I am an established patient; when do I need to make my annual appointment?
Some providers book out far in advance. Please schedule your annual appointment five to six months in advance.
Do I need to fast before I come to my annual appointment?
If you know prior to your appointment that you will have your fasting labs done (lipid panel, thyroid profile, metabolic panel etc.), fasting means you will avoid having to make a second visit. Specifically, fasting means consuming only water after midnight until the time of your appointment.
I just finished my appointment. When will my prescription be ready?
Prescriptions are sent to pharmacies electronically within two hours of your appointment. If your preferred pharmacy does not participate with e-prescribe, we will call in your prescription for you or provide you with a paper prescription. Check with your pharmacy on their specific turnaround times to fill your prescription, as it may not be ready right away.
What is your turnaround time for completing disability or FMLA paperwork?
We ask that you allow 10 business days for us to complete your disability paperwork.
